Internship assessment—general medicine department

intern of 2k17 batch 

was posted in department of general medicine on dates 02/01/23 to 03/03/23 including psychiatry  from dates 15 jan to 30 jan 

General medicine was my first posting in internship and reported enthusiastically on date and alloted unit 1 under pgs  dr. Prachethan and dr vivek sir . I took a little bit of time to cope up with process but got set by 1 week 

On the very first day i monitored blood transfusion whose hb was 6mg/dl until 9pm on non duty day, after that patient was taken over by obs gyn department for some gynaecological issues .

On op day there were nearly 70 pts and recorded nearly 30 patients vitals and among them few are admitted 

My very first case was very intrested pt presented with slurring of speech and decresed tone she got admitted and i took interest in that case from starting to end till dicharged . This case also helped me to diagnose my friends  mother who presented with same symptoms even before admitted into hospital.

Everyday at 8 am starts with updating soap notes and fever chat and posting in respective groups  and pajrs.

There were 2 acute pancreatitis cases admitted at same time with h/o alcohol intake . They were referred to psychiatric department after proper treatment and with help of rajkumar  sir (clinical psychologist) in kims made sure they got admitted in de addiction centre

And in 15 days i learned how to set up iv line , how to send samples and interpret results and abga  interpretations , also it was a hectic 2 weeks duration after that for 15 days i was posted in psychiatry 

It was like rest period from hectic unit duties in general medicine, there I learned how to take psychiatry history taking in detail and some interesting cases like schizophrenia , ocd , depression , etc . De addiction was full of cases those who wanted to quit alcohol and smoking . And also i had a good time to prepare for my entrance exam 

After 15 days i was posted in icu , nephrology and wards , monitored all patients and also ventilatory patients learned how to take abg samples inserted foleys catheters and assisted central lines and monitored dialysis patients , saw a patient who has psvt and learned drug of choice as adenosine and learned basic ecg findings,

And after i was posted in units for 15 days , every morning starts with updating of soap notes and fever charts and posting it in groups followed my pgs orders and discharged many patients with discharge summaries and  follow up medication ,
